Tratamento cirúrgico de ameloblastoma unicístico através de enucleação e aplicação de solução de Carnoy: relato de caso

Resumo

Introdução: O ameloblastoma é um tumor odontogênico de natureza epitelial, a mais frequente das neoplasias. Pode surgir dos restos da lâmina dentária, de um órgão do esmalte em desenvolvimento, do revestimento epitelial de um cisto odontogênico, ou das células da mucosa oral. O ameloblastoma é um tumor benigno, de crescimento lento, agressivo e infiltrativo com potencial de recidiva. Reabsorções radiculares, extrusões dentarias, assimetrias e dor são comuns. O tratamento destas lesões pode variar de enucleação associado a curetagem, associado a coadjuvantes químicos e mecânicos até ressecção com margem. Objetivo: O presente trabalho teve como objetivo relatar um caso de ameloblastoma, onde foi escolhida abordagem conservadora por curetagem e osteotomia periférica. Relato de caso: Paciente VHHL, gênero masculino, 36 anos, xantoderma, com queixas de dor, dormência no lábio inferior à direita, associado ao aumento de volume na região mandibular ipsilateral. A conduta inicial, foi a realização da punção aspirativa da lesão, após a confirmação histopatológica de ameloblastoma unicístico, foi realizada uma nova abordagem, uma enucleação associado a osteotomia periférica e aplicação da solução de Carnoy, o procedimento foi realizado sob anestesia geral. O paciente permanece em acompanhamento clínico e radiográfico, sem sinais de recidiva da lesão. Conclusão: O tratamento do ameloblastoma é dividido entre abordagem radical ou conservadora. No entanto, é importante examinar cada caso de forma minuciosa antes da escolha da abordagem cirúrgica, procurando, assim, obter um melhor prognóstico e ofertar a este qualidade de vida.
