Introduction: Proper treatment of malocclusions is essential, but it does not always happen. Objective: To investigate the use of orthodontic appliances in 12-year-old children. Methods: An inquiry was conducted with responsible for 410 schoolchildren in a municipality in the state of São Paulo. A questionnaire was used with the variables: use and type of appliance, reasons for installation, activation sessions, cost and satisfaction with the treatment, guidance on oral hygiene and frequency of tooth brushing. Results: Of the total, 15.86% used orthodontic appliances now of data collection or had a history of use, of which 64.62% were attended seen at private clinics, 16.92% at insured clinics and 15.28% at university clinics. As for the type of appliance, 60% were using or had a history of using fixed devices. Regarding the main reasons for the installation, 35.38% were due to aesthetics, 26.15% due to masticatory problems and 15.38% due to the dentist's indication. As for activations, 30% did not perform all sessions. The mean monthly costs of private treatments were R$72.98 and R$56.36 per insurance. Almost all those responsible (92.31%) reported that children had received guidance on oral hygiene and that 47.69% of them performed tooth brushing, on average, twice a day. As for satisfaction with the treatment, 10.77% of those responsible were dissatisfied with the cost. Conclusion: It was concluded that the use of orthodontic appliances was expressive, largely due to aesthetics, and that a considerable part of the access occurred through insurance and universities.
