Perfuração radicular lateral em um dente com calcificação pulpar: um relato de caso

Resumo

A calcificação pulpar pode representar um desafio ao tratamento endodôntico principalmente durante a abertura coronária.  A correta avaliação dos aspectos clínicos e radiográficos é indispensável para evitar a perfuração radicular. Este trabalho tem como objetivo mostrar por meio do relato de um caso clínico o selamento de uma perfuração radicular lateral com Agregado Trióxido Mineral (MTA). Paciente, 32 anos, sexo feminino, com queixa de dor leve ao mastigar no dente 25, desde que foi atendida na Unidade Básica de Saúde. Clinicamente, o dente 25 apresentava restauração provisória oclusomesial, não respondeu ao teste de sensibilidade pulpar, e o periodonto estava normal. Radiograficamente, havia sugestão de perda óssea mesial e comunicação com o ligamento periodontal. Na tomografia computadorizada de feixe cônico confirmou-se a presença de perfuração na face mesiopalatina, no terço médio, com perda óssea, comunicação com o ligamento periodontal e calcificação do canal radicular. Devido à presença de tecido de granulação, optou-se pela descontaminação e indução de reparo da área por meio da inserção de medicação a base de hidróxido de cálcio associado ao propilenoglicol.  Após 12 meses de trocas deste curativo, foi realizado selamento com MTA branco em íntimo contato com o periodonto. Realizou-se a blindagem coronária com cimento de ionômero de vidro e resina composta.  Apesar da localização e do tamanho desfavorável para o selamento desta perfuração, após 1 ano verificou-se no controle clínico-radiográfico-tomográfico que diante do tratamento executado o dente mostra-se em função, reparação óssea e assintomático.
