Treatment of Multiple Facial Fractures: Mandibular Fractures and Zygomatic Complex - Case Report

Abstract

The mandible, despite being a dense and resistant bone, because it is prominent in relation to the others, is one of the most fractured facial bones, followed by the nasal bones and the zygomatic complex. Men are more affected than women, and among the causes, car accidents, motorcycle accidents and physical aggression are the most frequent. Therefore, the objective is to report a clinical case of a patient victim of a motorcycle accident with mandibular and zygomatic complex fractures. A 26-year-old male patient, victim of a motorcycle accident, was admitted to the Oral and Maxillofacial Surgery and Traumatology Service of the Maria Aparecida Pedrossian University Hospital. Clinically, he presented edema in the left periorbital region, limited mouth opening, pain complaints, bone step on palpation in the infraorbital margin and left frontozygomatic pillar and altered dental occlusion. The tomography showed bilateral fracture of the condyle and left zygomatic complex. In view of the clinical and imaging data, a surgical procedure was performed under general anesthesia for reduction and osteosynthesis of fractures of the left zygomatic complex, associated with the installation of an Erich bar and maxillomandibular block for closed treatment of the condyle. Seen occlusal stabilization, conservative treatment of the condyle of the right mandible. The patient was followed up for a 6-month postoperative period without aesthetic and/or functional complaints. The treatment aims to promote adequate dental occlusion, reestablishment of mandibular movements and projection of the middle third of the face, something achieved by the proposed treatment.
