Abstract

The dimension of dentogingival structure is important in periodontal health. The increase in gingival tissue results in unfavorable smile esthetic and impairs periodontal health. Inflammatory gingival hyperplasia is a non-neoplastic proliferative process, which develops due to low intensity chronic irritating factors, such as dental biofilm. It is a relatively common sequelae in orthodontic treatment, as orthodontic appliances make hygiene difficult, causing an inflammatory process and consequently gingival hyperplasia. Cone beam computed tomography (CBCT), performed with lip retraction devices, allows the analysis of the dimension and relationship of dentogingival structures for surgical planning of gingival correction. The aim of this study is to report a case of surgical correction of inflammatory gingival hyperplasia after orthodontic treatment, with surgical planning using CBCT with lip retraction (ST-CBCT). A 23-year-old male patient came to the private office complaining of gingival disharmony in the upper arch. He reported having completed orthodontic treatment and performed supragingival scaling 30 days ago. In the intraoral examination, there was a gingival increase from tooth 14 to 24. With the ST-CBCT examination, the clinical diagnosis was inflammatory gingival hyperplasia, and the surgical planning was made based on the dentogingival measurements in the images. Gingivectomy and gingivoplasty were performed to restore the normal anatomofunctional characteristics of the protective periodontium. It is concluded that the ST-CBCT technique enabled diagnostic effectiveness and predictable surgical planning for smile line correction, with more precise aesthetic and functional results. Oral hygiene guidance is essential to maintain the results obtained.
