Odontoma as a Cause of Prolonged Retention of Primary Tooth: Clinical Case Report

Abstract

Prolonged retention refers to the persistence of a primary tooth in the oral cavity beyond the normal period for its exfoliation. Odontomas are benign odontogenic tumors, considered developmental anomalies rather than true neoplasms. The etiology of odontomas is associated with trauma, infections, or pressure, which can disrupt the genetic mechanism responsible for dental development due to the mutation of one or more genes. The aim of this report is to describe a case of prolonged retention of the tooth 51 caused by the presence of an odontoma in the anterior region of the maxilla. A 9-year-old male patient sought care at the Pediatric Dentistry Clinic of the Faculty of Dentistry of Araçatuba (FOA-UNESP) with a complaint of prolonged retention of tooth 51 and swelling in the area. Clinical examination revealed a slight bulge in the area of tooth 51, with no pain or spontaneous bleeding and with normal mucosal coloration. Radiographic examination revealed three radiopaque structures surrounded by a radiolucent halo located between the roots of the permanent upper central and lateral incisors. Initially, the diagnostic hypothesis was a dentigerous cyst, but the radiograph confirmed it to be a compound odontoma. The treatment performed consisted of the surgical removal of the lesion, with preservation of the adjacent dental structures. This case highlights the importance of preventive dental care to allow for less invasive treatment, especially since odontomas are asymptomatic and can lead to prolonged retention of primary teeth and impaction of permanent teeth.
